for Children with Special Health Care Needs

Public Health Nurse Case Manager

Provides a continuum of services designed for families of infants and children (prenatal to age 18) who have or are at risk for special health care needs. Services include early identification and tracking, screening and referral, nutrition assessment, information and linkage to community resources, transition from pediatric to adult care systems and health education. Limited financial assistance may be available for specialized medical care not covered by insurance or medical coupons.

The Children with Special Health Care Need Program at the Washington State Department of Health and Children’s Hospital in Seattle have designed a care plan survey for families of Children with Special Needs.
